Confirm the person has a severe and prolonged physical or mental impairmentThe impairment must be expected to last, or have lasted, at least 12 months.
Form T2201 (Disability Tax Credit Certificate) downloadedAvailable from canada.ca under Taxes / Disability Tax Credit.
Applicant's full name, address, date of birth, and Social Insurance NumberThe applicant is the person claiming the credit, often the caregiver or care recipient.
Care recipient's Social Insurance Number (if different from applicant)
Medical practitioner identified who can certify Part B of the formA doctor, nurse practitioner, optometrist, audiologist, occupational therapist, physiotherapist, psychologist, or speech-language pathologist depending on the impairment.
Appointment booked with medical practitioner to complete certificationBring the T2201 and any relevant medical records to the appointment.
Decision on submission methodSubmit online via CRA My Account using 'Submit documents,' or mail the signed form to your local tax centre.
Copy of completed form saved for personal recordsKeep a clear photocopy or scan before sending it to the CRA.
