How to Claim Carer Support in New Zealand: The Form and Process
Once you have a Carer Support allocation, claiming reimbursement involves a specific process — here’s exactly what’s needed.
Before you can claim
You need a completed needs assessment from a NASC service and a confirmed allocation of days/hours. If you haven’t done this yet, see our Carer Support overview guide first.
What you’ll need for your claim
- • The Carer Support Claim Form (available from the Health NZ or Disability Support Services website, or by requesting a physical copy by phone or email)
- • The disabled person’s client name and client ID
- • The full-time carer’s name
- • Receipts or supporting documentation for the respite costs being claimed
- • Bank account verification showing the full account number (including suffix) and account holder’s name, or an internet banking screenshot with these details, if submitting for the first time or updating your payment details
How to submit your claim
- • By email (fastest): send your completed form and supporting documents to csclaims@health.govt.nz — include the client name and/or ID in the subject line, not just on the form itself
- • By post: Health New Zealand, Private Bag 1942, Dunedin 9054 (a postage-paid address, no stamp required) — use an envelope if including receipts or extra paperwork
