Medicaid family caregiver pay checklist

1. Step 1

   Start from the right assumption: there is no single national program

   Medicaid pays some family caregivers in some states, under programs with different names, different rules about who can be paid, and different pay rates. What applies to you depends on the state the person you care for lives in.

2. Step 2

   Confirm the care recipient's Medicaid eligibility first

   Everything downstream depends on this. The person receiving care must qualify for Medicaid in their state before any caregiver payment route is open — this is the step to settle before spending time on program details.

3. Step 3

   Identify your state's program and its name

   Program names vary widely — self-directed care, consumer-directed services, home help, structured family caregiving. The five examples below show how differently the same idea is implemented.

4. Step 4

   Check whether your relationship is allowed to be paid

   This is the most common disqualifier and it is state-specific. Some states exclude spouses; some exclude parents of minor children; at least one explicitly allows spouses. Check this before anything else in the application.

5. Step 5

   Complete the assessment your state requires

   Every example below runs through an assessment of the care recipient's needs — by a state contractor, a county social worker, a managed care organization, or a case manager. The assessment usually sets the hours or level of care that can be paid.

6. Step 6

   Enrol as a paid caregiver through the required intermediary

   Most states route payment through a fiscal intermediary, a financial management services agency, a state provider system, or a contracted provider agency. Enrolment usually includes background checks and payroll paperwork before the first payment.
