FOR HEALTHCARE & CARE ORGANIZATIONS

Extend care beyond the visit.

FeatherKeep helps organizations extend practical support beyond hospitals, clinics and scheduled appointments — coordinating recovery support, patient follow-up, navigation and family care around life at home.

Build a defined program with FeatherKeep for the people your organization serves.

  • Health Systems & Providers
  • Health Plans & Insurers
  • Health-Tech & Care Organizations
  • Community & Public-Sector Care Programs

THE GAP AFTER CARE

The care journey continues after the appointment ends.

Discharge, referrals and care plans do not complete themselves at home. People may still need help understanding next steps, coordinating appointments, staying connected, navigating services and involving family or authorized supports.

01

Transitions from hospital or clinic to home

02

Follow-up between scheduled visits

03

Navigation across services and community resources

04

Family and caregiver coordination

FeatherKeep provides an operational support layer around these moments, within an agreed program scope.

FOR PROFESSIONALS

When someone you support needs additional help at home.

Healthcare handles clinical care. Home is where everyday life continues. If a person or family you work with may need non-clinical support at home — or help coordinating the practical transition between healthcare and everyday life — you can contact FeatherKeep. We will follow up, and support begins with an assessment and care plan agreed with the person.

  • Hospital discharge and transition professionals
  • Social workers and case managers
  • Medical practices and rehabilitation professionals
  • Retirement and independent-living communities
  • Disability, dementia and chronic-condition organizations
  • Maternity and postpartum professionals
  • Family, caregiver and seniors organizations
  • Churches, faith and cultural communities
  • Elder-law and financial professionals serving families
  • Employers and benefits organizations

Care workers currently available in Surrey/South Surrey, White Rock, Victoria, Nanaimo and Comox. We also accept requests in Langley and other Vancouver Island communities. All care is subject to assessment and a suitable local worker match. Worker availability in Comox does not mean workers are available throughout Comox Valley.

This list describes who may contact us; it does not indicate existing relationships. Please share only what is needed to start a conversation — personal and health details are gathered later through intake, with consent. Reach us at 604-916-0259 or admin@featherkeep.com.

PROGRAM OPERATIONS

The operational layer behind the program.

FeatherKeep can configure agreed workflows around the population, service model and responsibilities defined with each organization.

  • 01Referral & intake
  • 02Participant onboarding
  • 03Scheduled outreach & check-ins
  • 04Care navigation & service coordination
  • 05Appointment & follow-up support

  • 08Documentation & agreed reporting

Available capabilities depend on program scope, jurisdiction, staffing model and implementation requirements.

IMPLEMENTATION

From care gap to operating program.

  1. 01

    Define the population and need

Identify who the program is for, the care gap being addressed and the responsibilities FeatherKeep would take on.

  2. 02

    Design the workflow

Agree referral routes, outreach cadence, service boundaries, escalation procedures, documentation and reporting requirements.

  3. 03

    Launch a defined program

Begin with an agreed implementation model, population and service area.

  4. 04

    Review and evolve

Review service activity and agreed program measures, then refine the operating model as appropriate.

PROGRAM GOVERNANCE

Clear scope. Defined workflows. Accountable operations.

Program requirements are agreed during implementation. These principles guide the scope of a potential partnership, not a certification claim.

  • Defined service scope and responsibilities
  • Participant consent and authorized-contact workflows
  • Role-based operational responsibilities
  • Escalation and missed-contact procedures
  • Agreed service documentation
  • Program-specific reporting requirements

Service scope

FeatherKeep provides non-clinical support, coordination, navigation and practical care services within an agreed scope. We do not provide emergency services, clinical monitoring, diagnosis, treatment, nursing or medication administration. Clinical care and medical decisions remain with qualified healthcare professionals.
