Coming home from hospital can leave a gap

Discharge often happens quickly. The person may be tired, less steady, or following new instructions, while meals, laundry, appointments and everyday routines still need attention. Family members may live further away or be juggling work and their own households.

Hospital and community teams focus on medical care. FeatherKeep helps with the practical, non-clinical side of life at home during recovery — so the person and their family have less to hold together on their own.

Frequently asked questions

What support may someone need after leaving hospital?

It varies. Many people need practical help for a while — meals, laundry, getting to the shower safely with someone nearby, company, or someone to keep track of follow-up appointments. The Care Assessment looks at what the person actually needs and what family members can already cover.

Can FeatherKeep work alongside publicly funded home support?

Yes. Families may choose private home support alongside publicly funded home and community care when additional individualized support is needed. FeatherKeep is an independent private provider. We are not affiliated with, contracted by or endorsed by any health authority, and our support does not replace publicly funded services.

Can someone accompany me or my parent to follow-up appointments?

Appointment accompaniment is part of FeatherKeep’s support, subject to assessment and worker availability. A care worker can go with the person, offer company and help keep notes of what was discussed. We don’t provide transportation, make medical decisions or act as a clinical advocate.

Does FeatherKeep provide nursing care?

No. FeatherKeep provides non-clinical support. We do not provide nursing, wound care, medication administration, injections, clinical monitoring or medical decision-making. Those remain with the person’s healthcare team.

How does hospital-to-home support begin?

Call or request a callback to discuss practical support after discharge. We review local availability, complete an assessment and prepare a proposed Care Plan and arrangement with schedule and price. Your care needs, schedule, price and suitable local worker are confirmed before support begins. Planning before discharge day can help, when timing allows.

Can support change as recovery progresses?

Yes. Needs often change in the weeks after coming home. Support can be reviewed and adjusted — for example, reduced as independence returns, or changed if new needs appear. Changes to the Care Plan are confirmed with you first.
