Hospital-to-Home & Recovery Support in Surrey, BC
Coming home from hospital does not always mean everyday life immediately returns to normal. The discharge goes ahead, the person is glad to be home — and then the stairs, the shower and making dinner turn out to be harder than anyone expected, while family members are back at work.
Hospital-to-home support fills that practical gap in the first days and weeks: steady, non-clinical help at home while the person recovers, and one coordinated plan so the whole family isn’t improvising separately.

When families get in touch
Everyone’s recovery is different, but the moments that bring Surrey families to us are familiar ones:
- A parent is being discharged and will be weaker or less mobile for weeks — and lives alone
- A planned surgery, such as a hip or knee replacement, means limited mobility during recovery
- Family can help on weekends, but weekdays are the problem
- Follow-up appointments are coming up, and someone needs to be there to listen and take notes
- Showering, dressing and moving around the house will be difficult at first
- Meals, groceries, laundry and errands still need doing while energy is low
Practical help during recovery
Support is planned around the person’s recovery and any instructions their healthcare team has given:
Settling back in
Groceries in the fridge, laundry done, pathways cleared and the home easy to move around in for the first days back.
Meals and the household
Meals cooked, laundry kept up and light housekeeping handled while energy is low.
Personal care, where assessed
Temporary help with showering, dressing and moving safely around the home.
Check-ins and company
Regular visits that keep an eye on how things are going and make recovery less isolating.
Appointment accompaniment
Someone to go along to follow-up appointments, on transport the family arranges, and help the person remember what was said. We don’t provide transportation.
One coordinated plan
Updates and a single Care Plan, so everyone in the family knows who is doing what.
Recovery support is priced on the support the person is assessed as needing — for example, Personal Care at $65 per hour with a 2-hour minimum. You see the full price before approving.
Who recovery support is for
- Adults of any age returning home after a hospital admission
- People recovering from planned surgery, such as a knee or hip replacement
- Anyone living alone who expects the first weeks at home to be hard
- Families who want support organized before discharge day, not after
Organized before discharge day, useful after it
Ideally, we hear from you before the person leaves hospital. We’ll go over what the care team has said about moving and bathing, how the home is set up, and which days family can and can’t be there — then propose support that works around all of it.
Recovery rarely moves in a straight line. Support can start with more frequent visits and taper as confidence returns, or shift into ongoing help if that turns out to be needed. We keep following up and adjust the plan with you as the weeks pass.

Alongside the healthcare team, not instead of it
FeatherKeep supports the practical transition home. We don’t decide or arrange hospital discharge, and we don’t provide nursing, wound care, medication administration, physiotherapy, occupational therapy, clinical monitoring, transportation or medical decision-making.
Medical care stays with the hospital and the person’s own healthcare providers. Care workers follow the everyday guidance those teams have given, such as mobility precautions.
Recovery support in Surrey and South Surrey
Care workers are currently available in Surrey and South Surrey, British Columbia. All care is subject to assessment and a suitable local worker match.
If you or someone you care about is returning home after a stay or procedure at Surrey Memorial Hospital or another hospital, FeatherKeep can help coordinate non-clinical support for the transition home.
FeatherKeep is an independent private provider. Families arrange our support privately, and it can sit alongside any publicly funded home support the person receives.
Hospital-to-home support in Surrey: common questions
When should we set up support before discharge?
As soon as a discharge or surgery date exists. Support depends on assessment and care worker availability, so the more notice we have, the more options we can offer.
What kind of non-clinical help is available after hospital?
Meals, laundry and light housekeeping, help with groceries, assessed personal care such as showering, check-in visits, appointment accompaniment and family coordination — all arranged in one Care Plan.
Can someone accompany the person to follow-up appointments?
Yes, after assessment and subject to care worker availability. The care worker can go along on transport the family arranges and help the person remember what was said. We don’t provide transportation.
Can support be temporary during recovery?
Yes. Visits can begin frequently and taper as strength returns, or settle into lighter ongoing support — the plan follows the recovery, not a fixed term.
Can someone help with showers while my parent recovers?
Yes, when personal care is in the Care Plan. Care workers work to the mobility and bathing precautions the person’s healthcare team has set out.
Do you provide wound care or give medications?
No. Both are clinical tasks outside non-clinical support. The healthcare team will arrange them if they’re needed.
Do you coordinate with the hospital’s discharge team?
We’re independent and not part of any hospital’s discharge process. Families share the instructions they’ve received, and the Care Plan reflects them.
