Hospital-to-Home & Recovery Support in White Rock, BC
The discharge date arrives, sometimes sooner than expected. The person is ready to leave hospital, but the stairs, the shower and making a meal still feel like a lot — and family can’t be there every hour.
Hospital-to-home support fills the practical gap in those first days and weeks, so recovery can happen at home with steady help around the parts that are temporarily harder.

Common moments when families look for help
Whether someone is coming home from Peace Arch Hospital or another hospital, the questions are often the same:
- Your parent is coming home after a fall, illness or hospital stay and will be weaker for a while
- A hip or knee replacement, or other planned surgery, means limited mobility for several weeks
- Showering and dressing will be difficult at first
- Family can help some days, but not all of them
- Follow-up visits with the surgeon or family doctor are coming up, and nobody is sure who can go
- Several people are trying to coordinate meals, visits and errands, and it’s getting complicated
Practical help during recovery
Support is planned around the person’s recovery and any instructions they’ve been given by their healthcare team.
Settling back in
Help with groceries, tidying and making the home easier to move around in the first days back.
Meals and household tasks
Simple, nourishing meals, laundry and light housekeeping while energy is low.
Personal care, where assessed
Temporary help with showering, dressing and moving around safely at home.
Check-ins and company
Regular visits to see how things are going and make the days less isolating.
Appointment accompaniment
Someone to go with the person to follow-up appointments, on transport they’ve arranged, and help them remember what was said. We don’t provide transportation.
Coordinating with family
Updates and a single plan, so everyone knows who is doing what.
Recovery support is priced on the support assessed — for example, Personal Care is $65 per hour with a 2-hour minimum. You see the full price before approving.
Who hospital-to-home support may help
- Older adults returning home after a hospital admission
- People recovering from planned surgery such as a joint replacement
- Someone who lives alone and needs a few weeks of practical help
- Families who want support in place before discharge day
Planning around the recovery, not just the discharge
The best time to talk to us is before the person leaves hospital. We’ll ask what the hospital team has said about mobility, activity and personal care, what the home is like, and who in the family can help when — then propose support that fits around that.
Recovery changes week by week. Support can start with more hours and taper as the person regains confidence, or shift towards ongoing help if that turns out to be needed. We follow up and adjust the Care Plan with you.

Alongside the healthcare team, not instead of it
FeatherKeep supports the practical transition home. We don’t decide or arrange hospital discharge, and we don’t provide nursing, wound care, medication administration, physiotherapy, occupational therapy, clinical monitoring, transportation or medical decision-making.
The hospital and the person’s healthcare providers remain responsible for medical care. Care workers follow the everyday instructions they’ve given, such as mobility precautions.
Recovery support in White Rock
Care workers are currently available in White Rock. All care is subject to assessment and a suitable local worker match.
If you or someone you care about is returning home after a stay or procedure at Peace Arch Hospital or another hospital, FeatherKeep can help coordinate non-clinical support for the transition home.
FeatherKeep is an independent private provider and is not affiliated with, endorsed by or contracted by Fraser Health or any health authority unless a specific future contractual relationship is established. Families arrange our support privately, and it can sit alongside any publicly funded home support the person receives.
Hospital-to-home support in White Rock: common questions
When should we arrange support before someone comes home?
As early as you can — ideally once a discharge or surgery date is known. Support depends on assessment and care worker availability, so earlier planning gives more options.
Can support continue while someone recovers?
Yes. Many families start with more frequent visits and reduce them as recovery progresses. Others find ongoing support helpful and continue.
Can someone go with the person to a follow-up appointment?
Appointment accompaniment can be arranged after assessment, subject to care worker availability. We don’t provide transportation, so the person or family arranges the ride.
Can a care worker help with showering after surgery?
Yes, when personal care is included in the Care Plan. Care workers follow the mobility and bathing precautions the healthcare team has given.
Can you change dressings or give medications?
No. Wound care and medication administration are clinical tasks we don’t provide. The healthcare team will arrange these if needed.
Do you work with the hospital discharge team?
We’re independent and not part of any hospital’s discharge process. Families share the instructions they’ve received so the Care Plan reflects them.
Is this only for older adults?
No. Adults of any age recovering at home after hospital or surgery can request support.
